You can print this page for reference as you prepare your request!

Your request must include ALL of the following information.  Our online application requires you to attach several documents including budgets and a board list.  We prefer documents that are prepared with Word, Excel, pdf, or other compatible programs.

  • If you are requesting funds for a Charter School, click here.
  • List of Board of Directors; must include term of service, leadership roles, corporate affiliations and other non-profit boards served on.
  • Compensation and length of employment for five highest paid employees.
  • Current budget for entire organization AND for specific project as per request. You can use our templates or your own, as long as all details are provided.

  • Total dollars personally contributed by the members of the Board of Directors (exclude the value of in-kind goods or services, matching gifts from corporate giving programs or gifts secured by the board member)

  • Percent of board members who make personal financial contributions to the organization
  • Range of personal financial contributions from board members (i.e., lowest gift and highest gift)

  • Minimum personal financial contribution required from each board member, if any
  • Minimum financial "give or get" obligation for each board member, if any

If you are seeking general support, this information should reflect the entire agency's annual fundraising budget. Please specify individual, corporate, foundation, board of directors, government, investment, fees for service and fundraising/special event income amounts in your budgets.  If your grant includes a challenge or match, please describe how you expect to meet the goal.
  • Proof of IRS 501c3 status - our online grant application will verify your non-profit status with the IRS - no other written proof is needed at this time.
  • We may request a copy of your most recently filed IRS form 990.
  • We may request a copy of your most recent reviewed or audited financial statements.
  • We encourage you to use documents and exhibits you already have available.

Annual Grant Evaluations

Grantees must provide an annual grant evaluation report. Your organization must send a written report showing how the grant money was used and an internal evaluation of the program. New grant requests will not be accepted or reviewed by the Foundation until your report has been filed. You may download this suggested format for your report. Be sure to note the due date listed in your grant agreement on your calendar.

ITEMS TO NOTE

  • As a part of the Foundation's grant review process, a board member or employee of the Foundation may visit a grant applicant.
  • The Foundation also makes discretionary grants to organizations selected by the Board members.
  • Board members may authorize grants in between meeting dates. These grants should be of an urgent nature, where time is of the essence to assist in a unique situation. These grants are not a substitute for the regular grant making process of the Foundation.
  • The Foundation's Board of Directors meets several times each year to review grant requests.
  • If a grant request is denied for any reason, your organization may not submit another application until 12 months from date of denial. Reapplication on a denied request is not encouraged.
  • Organizations that seek renewed support must wait until 12 months from date of previous grant award, and must submit a complete request as per these guidelines.

GRANT DEADLINES

We do not have specific deadlines for grant applications.  Our board meets several times each year.  Grants are reviewed in the order they are received.  Most grant applicants will hear from us within 90-120 days of application date.
