About our electronic grant application:
You must be an IRS registered non-profit, with a valid Federal ID number. Our system is linked to the current IRS database; no additional proof of non-profit tax status is necessary. If your IRS determination letter is dated within the past 6 months, our system may not recognize your ID number. If this is the case, please fax (847-716-2177) or email (susan@cosfoundation.org) a copy of your IRS determination letter and request access to the application. We will respond by email with a special link to the application. Please note that historically, we have not supported start-up organizations or faith-based organizations operating under a group exemption.
You can use documents already prepared in your word processing and/or spreadsheet software - just "cut and paste" into the grant application. You may be able to do the same from information posted on your agency's website as well.
Your application is not complete until you attach three required documents - a board list, your agency budget, and the project budget. If you are unable to attach documents, please contact our office for alternative submission arrangements.
If you need to leave and return to a partially completed application, please sign up as directed. You will then receive an automatic email response with a link to your application. This is the only way to return to your application.
You will have an opportunity to review and print a copy of your application prior to submitting it to our foundation.
We may contact you upon receipt of your application to request additional supporting documents.
